• Supporting information either emailed to This e-mail address is being protected from spambots. You need JavaScript enabled to view it , dropped in at  Suite 15 Forster Tower, Wallis Street or posted to PO Box 380, Forster by 5pm on Friday 30 September.
• A separate entry form and supporting information for each category you have entered
• Completed the ‘Business of the Year’ question on ALL nomination forms (please just copy and paste from other forms if you enter more than one category).
• A copy of your logo and 3 or 4 jpg images showing you and your team and/or products or customers of at least 300KB in size
